This paper presents and emphasizes to demonstrate the impact of human errors and common cause shock failures (CCSF) in reliability analysis of a three unit identical system. Various measures of system effectiveness such as time-dependent availability, steady-state availability and frequency of failures of the system are obtained. The failed units are repaired singly and repair times are exponentially distributed. The numerical illustration is provided in order to highlight the effect of human errors and common cause shock failures on the system. |
